Family Medical Clinic - Ubi Caritas State Funded Rehab

Our dental services are provided to people living in the Beaumont Community. Services include basic dental care, assessment and evaluations, x-rays, extractions and fillings. We are unable at this time to provide dentures.

Office Hours
Office hours are generally from 10:00 AM to 5:00 PM on Monday thru Friday. If our receptionist is busy, our voicemail system will answer. Please leave a message and we will return your call.

Eligibility Guidelines
We serve the Beaumont community; you must live in one of the following zip codes; 77701, 02, 03, 04, 05, 06, and 07. When you arrive for your first appointment at the Clinic, you will be asked to provide proof of residency, such as a driver's license, water bill, or vehicle registration.

As a 501(c)(3) registered not-for-profit organization, we are required by law to show documentation of income in our patients' files. You must provide proof of total household income, preferably a current tax return, in order to be treated. Other examples of proof of income include Form 1099s, paycheck stubs, and disability forms. When you call the Clinic for the first time, we will help you determine whether or not you fall within the income guidelines.

We cannot serve any patients who are on Medicaid or covered by any type of dental insurance.

Be prepared to describe your current dental problem accurately. We will also ask if you have any other medical problems that may affect our ability to treat you.

Getting an Appointment
We take patients by appointment only, on a first come, first served basis, with priority given to emergencies. At any given time, there are approximately 100 people on our waiting list. When you call, we will take your information and place you on the waiting list. You will be notified as soon as we have an opening. Please be patient with us.

Fees
We may ask you to participate in your dental care by paying a small portion of the cost of materials used in your procedure. This is normally no more than $20.00 and if there is a particular hardship this can be waived.
Appointments
Our Dentist, Hygienist and dental assistants are volunteers. Your appointment is very important and you must make the appointment or notify the office 24 hours in advance if you need to reschedule. A missed appointment means that the dental professional may have wasted their time, so please help us by being on time for your appointment.

This dental assistance location provided is subject to change. We work to maintain an accurate list, but we urge you to call the location directly for further information as may have changed since this posting was created.
