People's Community Health Center State Funded Rehab

Is it free? Think of it as a partnership. In addition to providing your medical care, we can help you get the benefits to pay for that care. And by providing us with information we need, you can help us keep our costs under control. We don't turn away patients because of their inability to pay. But we expect patients to partner with us to help us get compensated for the care we provide. What kind of information does People's need from patients? Much of our funding comes from the government. That's why compensation can be a complicated issue. Still, we're happy to work with you to make sure you get the benefits you're entitled to. If you're employed, please bring copies of pay stubs. If you're not employed, but have worked in the last six months, we'll need one of these: your information from the state unemployment office, your Social Security award letter or a letter from your former employer stating the last date you worked. Next, if you're a US citizen, please bring us a photo ID and a document that proves you were born in this country: a birth certificate, a passport - current or expired - or a certificate of citizenship or naturalization. If you're not a US citizen, please bring us proof of your address and proof of your income.
